Managing Your Health Profile
Your Health Profile is the foundation of VitalSync's Personal Health Report. The Medical & Surgical History section of your PDF report is drawn directly from this profile — it is only as complete as the information you enter here.
How to Open Your Health Profile
Medical History
Enter any conditions you have been formally diagnosed with by a healthcare provider.

| Condition name | Hypertension, Type 2 Diabetes, Asthma |
| Diagnosed date | Month and year (approximate is fine) |
| Managing provider | Your GP or specialist's name |
| Status | Active / In remission / Resolved |
Tip: Include chronic conditions even if they feel well-controlled. Your provider needs the full picture, not just active problems.
Surgical History
Record any past surgical procedures.

| Procedure name | Appendectomy, Knee arthroscopy |
| Date | Year is sufficient if exact date is unknown |
| Hospital / Facility | Optional but helpful |

Allergies
List all known allergies, including medication allergies, food allergies, and environmental allergies.

Important: Medication allergies are especially critical. Always keep this list current, particularly before any new prescription or procedure.
Medications & Allergies — Current Medications
This section captures your full medication history, not just the medications tracked by Smart Reminders.

Note: Medications added to Smart Reminders are tracked for adherence (taken / skipped / missed). Medications entered in the Health Profile appear in the Personal Health Report's Medical History section. Both lists are included in the full PDF report.
How This Affects Your Personal Health Report
When you generate a Personal Health Report PDF, the Medical & Surgical History section is populated entirely from your Health Profile. If a field is empty, that section will show “No data entered” in the PDF.
Before your next appointment, review each sub-section and confirm:- [ ] All active conditions are listed with correct status.
- [ ] Any surgeries in the past 5 years are recorded.
- [ ] All medication allergies are entered and marked with the correct severity.
- [ ] Any medications changed since your last visit are updated.
